Birthday Party Waiver of Liability

WAIVER OF LIABILITY
I, for myself, my child or ward sign this Waiver and Assumption of Risk in consideration of the opportunity to use the facility, or to participate in any parties or activities at/by Velocity Sports Performance.  I, for myself, my child or ward acknowledge and understand that there are dangers and risks associated with the activities at/by Velocity Sports Performance and agree to assume all risk of personal injury, including the potential for paralysis and death.  I, for myself, my child or ward agree to follow the safety instructions provided and acknowledge that failure to do so may result in expulsion from Velocity Sports Performance.  I, for myself, my child or ward, and on behalf of my or their heirs, assigns, personal representatives and next of kin, Herby Hold Harmless Velocity Sports Performance, its owners, members, officers, employees, equipment manufactures and sponsoring agencies from all liability for any such personal injury, disability, death or loss or damage to person or property to the fullest extent of the law.
 
I understand this document is a contract.  I have read it thoroughly.  I understand and agree to the terms and conditions it contains.

From 17 South:
Take 17 South
**Right on Corporate Drive (just past Robbie’s Music Barn)
Immediate left into our parking lot, you will see our sign on right.
 
From 17 North:
17 North to Macarthur Blvd exit
Left at light, going over 17
Follow half way up the hill to Corporate Drive, Go right.
Follow this to the end and take a right into the parking lot of Commerce Center (last building before you hit 17).
 
From 287North:
287 north to the 17 South exit.
Follow the exit up and over 287 then back down on to 287 south. Merge into the left lane and get off on 17 South.

From the New York Thruway:
Follow 87 South to 287 South
Take the 17 South exit and follow the directions above.
